Job description

Transitional Services for New York, Inc., (TSINY) not-for profit, is a comprehensive, community-based mental health organization located in New York City. We provide a continuum of rehabilitative services to enrich the lives of those recovering from mental illness and facilitate their transition to increased levels of independence. Transitional Services for New York, Inc. envisions broadening its rehabilitative services and becoming a regional social service provider. Transitional Services for New York Staff will deliver effective programs with compassion, integrity, and professionalism. We expect all staff to put our clients’ needs first while respecting ourselves and each other as we provide hope to those who participate in our programs.

TSINY is looking for Full-Time Program Coordinator for our PROS Location, a Rehabilitative and Recovery program located in Jamaica (Queens County), NY.

Program Coordinator: $70,000 annually

Master’s degree in social work, mental health counseling, rehabilitation counseling, psychology or related field plus licensure and three years PROS experience

Position Summary

Responsible for the daily group operations and activities of the program ensuring the placement of participants in medical necessary groups and that groups are facilitated along acceptable standards. Responsible for internal QA responsibilities ensuring that documentation of services to participants are in compliance with regulatory, PROS standards of care and Agency’s guidelines.

Essential Functions

  • Work closely with the Program Director in the development of groups and curriculum.
  • Assign staff to facilitate groups.
  • Provide guidance and training to staff in the area of effective group facilitation.
  • Oversee the assignment of participants to groups.
  • Assign alternative group facilitators in the absence of staff.
  • Post daily group schedules.
  • Maintain the high-risk list in accordance with the Standards of Care and the High Risk policy.
  • Assists the Program Director and the Assistant Program Director in the authorization process.
  • Follow-up on State and internal audits ensuring that corrective plans are implemented and followed.
  • Review charts and AWARDS to ensure compliance with regulatory and Agency guidelines.
  • Oversee the completion of URs.
  • Provide CT services when required.
  • Follow-up on hospitalized participants.
  • Maintain a limited caseload of participants when necessary.

  • Require manual dexterity for the use of a computer, telephone, fax, or copy machine.
  • Be able to prepare clear, concise reports.
  • Be able to communicate effectively orally and in writing.
  • Support the mission, values and vision of the organization.
  • Promote positive public relations with participants' support system (families, landlords, probation officers, etc.).
  • Complete requirements for in-service training, acceptable attendance, dress codes and personal hygiene.
  • Be able to solve complex problems and deal with a variety of issues.
  • Possess the ability to effectively present information and respond to questions from participants, managers, families, professionals and the general public.
  • Possess ability to communicate sensitive information to principals and clients.
  • Be able to lift items up to 25 pounds.
  • Be able to sit or stand as needed, with or without reasonable accommodation.
  • May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation.
  • Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation.
  • During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
  • Attend job and Agency trainings as required to maintain compliance and ensure continued professional development.
  • Perform other related duties as required.

Experience/Education/Skills/Abilities

Master’s degree in social work, mental health counseling, rehabilitation counseling, psychology or related field plus licensure and three years PROS experience.
