Job description

The Mission

Do you believe that everyone deserves a vibrant social life and a supportive home environment? We are looking for a Community Support Mentor to join our team. This isn't a "sit and watch" kind of job. You'll be the bridge between our clients and their communities, acting as a coach, a companion, and a professional "wingman" for navigating life.


The Two Sides of the Role

Your day-to-day will focus on two primary service areas:

  • In-Home Respite
    • Providing high-quality care within the client's home. You'll help with daily routines, engage in hobbies, and provide primary caregivers with the peace of mind they need to recharge.
  • Community Networking
    • Getting out there! You'll help clients identify interests (like clubs, sports, or volunteering), attend local events together, and build the social skills needed for lasting independence.


What You'll Actually Do

  • Be a Social Architect: Help clients discover local events, classes, or groups that align with their passions.
  • Provide Meaningful Respite: Create a safe, fun, and engaging environment at home-whether that's playing board games, meal prepping together, or simply hanging out.
  • Bridge the Gap: Actively facilitate introductions and interactions in the community until the client feels confident doing it solo.
  • Documentation: Keep brief, clear notes on progress and goals (nothing fancy, just the facts).

  • Relationally Minded: You're a "people person" who knows how to build trust and maintain professional boundaries.
  • Adaptive: You can read the room. You know when to step in and help and when to step back and let the client lead.
  • Reliable: Our families and clients count on you. Consistency is your middle name.
  • Resourceful: You know your way around the local community (or you're a pro at using Google to find the coolest local spots).


Administrative Note

  • Scope of Work: Perform other duties as assigned by Managers or Directors to support the evolving needs of the individuals and the agency.
  • Operational Flexibility: The expectation to work additional hours beyond your primary schedule, availability between 7:00 am to 7:00 pm, to meet the needs of our clients, address emergencies, or ensure operational excellence.


The Must-Haves:

  • Education: High school diploma or equivalent.
  • Credentials: Valid Driver's License (Class E), reliable transportation, and a clean driving record.
  • Compliance: Must pass a comprehensive background check (FCSR/EDL/OIG/SOR).
  • Certifications: Current CPR/First Aid certification, possibly obtaining the L1MA (or willingness to obtain certification upon hire).
