Key Responsibilities

As an LPC at St. Charles Counseling Services, you will:

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues.
• Conduct comprehensive patient intakes that focus on diagnostic assessments and functional evaluations, creating tailored treatment recommendations.
• Provide brief, targeted therapeutic interventions for patients requiring mental health assistance.
• Identify patients who would benefit from transfer to specialty mental health facilities and advise the primary care team accordingly.
• Initiate follow-up communications to assess patient progress and reevaluate treatment approaches as needed.
• Develop relapse prevention strategies for patients and assist them in maintaining stability in their personal lives.
• Monitor and coordinate health services delivery related to behavioral healthcare, fostering collaboration with other providers.
• Document patient progress and maintain clinical records with a high degree of accuracy.
• Participate in interdisciplinary team discussions, contributing insights for effective treatment planning.
• Educate patients about self-management techniques and lifestyle changes to enhance their recovery.
• Comply with the Code of Ethical Conduct for Clinical Professionals and adhere to all organizational policies.

Patient Population / Client Focus

Qualifications

Preferred Experience
