Job Summary
Valeo Home Health is seeking skilled and compassionate Registered Nurses to provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and overall well-being.
Duties/Responsibilities:
  • Perform patient assessments and ongoing evaluations.
  • Implement physician-ordered plans of care and complete required documentation (OASIS).
  • Provide in-home nursing care for multiple patients per day.
  • Educate patients and caregivers on medications, disease processes, and in-home care.
  • Coordinate patient services and supervise LPNs/HHAs as needed.
  • Communicate changes in patient condition to physicians and care teams.
  • Maintain compliance with regulations and agency policies.

Physical Requirements
  • Ability to sit, stand, and drive for extended periods while providing in-home care.
  • Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
  • Regular use of a tablet for documentation, communication, and training.

  • Current up to date TB test.
