About the Role


We are seeking skilled and compassionate Registered Nurses (R.N.s) to join our growing team across the Bay Area. The ideal candidate is clinically strong, patient-centered, collaborative, and committed to delivering care that is thorough, responsive, and never rushed. As an AlphaCare RN, you will play a critical role in helping patients recover safely at home while coordinating care across physicians, therapists, caregivers, and support staff. This role includes completing OASIS documentation, developing individualized care plans, supervising LVNs and HHAs, and helping patients and families navigate care confidently in the home environment. At AlphaCare, success is measured not only by compliance — but by outcomes, responsiveness, trust, and the quality of the patient experience.


Qualifications


  • Current California Registered Nurse (R.N.) license required
  • Minimum of 1 year of nursing experience as an R.N. required
  • Home health experience preferred
  • Experience with OASIS documentation preferred
  • Strong clinical assessment and care coordination skills
  • Ability to work collaboratively and positively with patients, families, physicians, and interdisciplinary teams
  • Excellent communication, organizational, and documentation skills
  • Multilingual abilities are strongly valued


Responsibilities


As an AlphaCare RN, your role is to deliver more than task-based care. You are responsible for creating a patient experience built on clinical excellence, communication, dignity, and trust.


Responsibilities include:

  • Conduct initial home care visits and regularly reassess patient needs and progress
  • Develop and implement individualized care plans based on physician orders
  • Complete OASIS assessments for admissions, transfers, resumptions of care, re-certifications, and discharges
  • Observe, assess, and document patient symptoms, responses, and progress
  • Educate patients and caregivers on disease management, medications, treatment plans, and home healthcare techniques
  • Coordinate patient services and scheduling across the care team
  • Supervise and support LVNs and HHAs in the delivery of quality patient care
  • Communicate proactively regarding changes in patient condition with physicians, Clinical Managers, therapists, and case managers
  • Facilitate collaborative case conferences to ensure continuity and quality of care
  • Maintain timely, accurate, and compliant clinical documentation
  • Discharge patients from skilled nursing services when discharge criteria are met
  • Participate in continuing education, in-service training, and professional development programs
  • Comply with all agency policies, procedures, Medicare, and Medicaid regulations
