Base Annual Pay: Approximately $52,000
Potential Total Annual Compensation: $84,800+
Job Overview
Milenis Home Care is hiring a full-time Referral Partner Manager to develop referral relationships and generate new home-care clients throughout the Treasure Coast.
This is a field-based business development and community outreach position. You will spend significant time visiting healthcare organizations, developing professional relationships, attending networking events, following up with referral sources, and generating qualified client referrals.
We are looking for someone who is energetic, professional, persistent, organized, and comfortable being accountable for measurable results.
Responsibilities
  • Develop referral relationships with hospitals, rehabilitation centers, assisted living communities, skilled nursing facilities and physician offices
  • Build relationships with case managers, social workers, discharge planners, nurses, physicians and care managers
  • Develop partnerships with elder-law attorneys, estate planners, financial professionals, reverse-mortgage professionals, real estate professionals and community organizations
  • Conduct regular in-person visits to referral partners
  • Prospect for new referral sources
  • Maintain consistent follow-up with existing referral partners
  • Attend healthcare, senior-care and community networking events
  • Present Milenis Home Care services to professionals and prospective families
  • Generate qualified new client referrals
  • Maintain an active referral pipeline
  • Track visits, calls, referrals, admissions and results
  • Collaborate with Milenis operations and clinical leadership

Compensation
Base Pay: $25/hour
Approximately $52,000/year
Transportation Allowance: $400/month ($4,800/year)
Qualified Client Referral Bonus: $200 per qualified new client
A qualified client must begin services with Milenis Home Care for at least 20 hours per week.
Monthly Performance Bonus:
  • 3-5 qualified new clients: $500
  • 6-8 qualified new clients: $1,000
  • 9+ qualified new clients: $1,500

Annual KPI Bonus: Up to $10,000
Potential Total Annual Compensation: $84,800+ plus additional qualified-client referral bonuses
Performance-based compensation is not guaranteed and depends on achievement of established referral, admission and company performance goals.
Qualifications
  • Business development, sales, healthcare marketing, community outreach or referral-development experience preferred
  • Home care, senior living or healthcare experience strongly preferred
  • Existing professional network within the Treasure Coast is a plus
  • Excellent communication and relationship-building skills
