Responsibilities

As a Home Health Registered Nurse (RN), you will lead and manage patient care from admission through discharge—bringing clinical expertise, compassion, and coordination to every interaction. You’ll serve as the central point of contact for patients, families, physicians, and the interdisciplinary care team, ensuring care plans are executed safely, effectively, and with a focus on quality outcomes. In this role, you’ll combine hands-on nursing care with care coordination, education, and clinical oversight to support each patient’s unique journey.

Home Health Registered Nurse (RN) Responsibilities

  • Complete comprehensive patient assessments, including history, condition, and medication review
  • Develop and manage individualized care plans with clear, measurable goals
  • Deliver and coordinate nursing care in alignment with physician orders
  • Monitor patient progress, reassess needs, and adjust care plans accordingly
  • Communicate patient status changes and collaborate with physicians and care teams
  • Educate patients, families, and caregivers on care plans and ongoing needs
  • Coordinate interdisciplinary services and connect patients with additional resources
  • Supervise and support paraprofessional staff to ensure quality care delivery
  • Maintain accurate, timely documentation of all patient care activities
  • Participate in team conferences, quality improvement, and ongoing education
  • Ensure compliance with clinical standards, regulations, and safety practices
  • Support a collaborative, patient-centered care environment
  • Participate in on-call and rotating schedules as needed

Home Health Registered Nurse (RN) Qualifications

  • Mississippi Registered Nurse (RN) license
  • At least one year of nursing experience
  • Reliable transportation, valid driver’s license, and appropriate auto insurance
