What You'll Do
  • Deliver evidence-based primary care services in a patient-centered environment.
  • Build trusted relationships and help patients achieve their health goals.
  • Partner with patients and the care team, including a Whole Health Coach, to develop personalized care plans.
  • Provide preventive care, acute care, chronic disease management, and care coordination.
  • Participate in team meetings and quality improvement initiatives that drive exceptional patient outcomes.
Qualifications
  • Current, unrestricted NP or PA license (or ability to obtain prior to start)
  • Board Certification as an NP or PA
  • Current DEA registration
  • Primary care, family medicine, or internal medicine experience preferred
  • Passion for relationship-based, whole-person care
