Position Summary
The Private Duty Registered Nurse (RN) provides skilled nursing care to clients in their homes in accordance with the physician's orders, the plan of care, and agency policies. The RN works closely with clients, families, and the interdisciplinary care team to ensure high-quality, safe, and compassionate care in a home-based setting.
Key Responsibilities
  • Deliver direct patient care, including assessments, medication administration, treatments, and monitoring.
  • Develop, implement, and update individualized care plans in collaboration with physicians and the care team.
  • Monitor and document the client's condition, reporting significant changes to the physician and agency.
  • Provide patient and family teaching related to disease process, treatment, medications, and home care needs.
  • Ensure infection control and safety practices are followed at all times.
  • Coordinate with caregivers, therapists, and other healthcare providers to ensure continuity of care.
  • Maintain accurate and timely clinical records in compliance with agency policies and state/federal regulations.

Qualifications
