Job description

Home Health Registered Nurse High Tech Visits (Broward County FL,)
AccordCare is a licensed private duty agency seeking experienced RNs for hourly Hi-Tech private duty cases
Key Responsibilities
  • Complete Start of Care (SOC) admissions and comprehensive nursing assessments in patients' homes
  • Develop, implement, and update individualized Plans of Care based on patient needs, physician orders, and clinical findings
  • Perform supervisory visits and ongoing patient evaluations as needed
  • Provide safe, effective skilled nursing care within the RN scope of practice
  • Administer medications, treatments, and other skilled nursing procedures as ordered
  • Assist patients with mobility support, transfers, positioning, catheter care, and other clinical needs
  • Accurately document assessments, interventions, and patient progress in a timely manner
  • Promptly communicate any changes in patient condition, concerns, or emergencies to the Clinical Manager or appropriate healthcare provider
  • Coordinate patient care with physicians, therapists, caregivers, and interdisciplinary team members
  • Travel throughout the Treasure Coast area to complete home visits as assigned
  • Maintain professionalism, empathy, attention to detail, and compliance with all agency policies and regulatory standards
Qualifications
  • Active Florida Registered Nurse (RN) license in good standing
  • Previous home health and Start of Care experience preferred
  • Strong assessment, care planning, and clinical documentation skills
  • Ability to work independently and manage time effectively
  • Reliable transportation and valid driver's license required
  • Excellent communication and interpersonal skills
  • NEGATIVE TB Test / NEGATIVE QuantiFERON Blood Test
Preferred Experience
  • Experience with OASIS documentation
  • Familiarity with home health EMR systems such as Kinnser/WellSky
  • Knowledge of Medicare home health regulations and compliance standards
