Job description

Transitional Care Pharmacist

Serves as the Transitional Care Pharmacist for Bureau of Prisons' (BOP) inmates transitioning from correctional institutions to Residential Reentry Centers (RRC) and/or Home Confinement (HC). Responsible for the accurate and timely management, development, organization, and implementation of programs and procedures necessary to ensure adequate medically necessary healthcare is available for community designated inmates. Responsible for regular communication with community contract providers, RRC personnel, RRC residents, BOP physicians and healthcare providers, Pharmacy Benefit Management (PBM) providers and Contracting Officer Representatives (COR), Regional Community Treatment Coordinators, and Community Treatment Service (CTS) providers, and BOP pharmacy fill site staff to ensure all aspects of community based medically necessary healthcare is appropriately available and timely. Along with all other correctional institution employees, incumbent is charged with responsibility for maintaining security of the institution. The staff correctional responsibilities precede all others required by this position and is performed on a regular and recurring basis.
