Job Type: Full-Time / Part-Time / PRN (Flexible)

Responsibilities

As a Home Health RN, you will:

  • Provide skilled nursing care in clients’ homes

  • Perform assessments and develop individualized care plans

  • Administer medications, injections, and treatments as ordered

  • Monitor patient conditions and report changes to physicians

  • Provide wound care, catheter care, and chronic disease management

  • Educate patients and families on care, safety, and medications

  • Complete documentation accurately and timely

  • Coordinate care with caregivers, therapists, and other team members

Qualifications

  • Active Ohio RN License (required)

  • CPR certification (required)

  • Home Health experience preferred (but not required)

  • Strong communication and documentation skills

  • Reliable transportation and valid driver’s license

  • Ability to work independently and professionally
