Physician - Long-Term Care & Transitional Care Units (New Graduates are accepted!)

Location: Prescott, WI

Job Type: Full-Time/Part-Time (On-site)

Schedule: Mon - Fri, 9:00 AM - 5:00 PM (with on-call rotation)

Medical Specialties: Internal Medicine/Family Medicine/Geriatrics

Job Overview:

We are seeking a dedicated Physician to provide medical care for patients in Long-Term Care (LTC) and Transitional Care Units (TCUs) in the Prescott/St. Croix Falls, WI area. The provider will ensure high-quality, patient-centered care for residents in long-term and post-acute care settings.

Key Responsibilities:

  • Perform comprehensive initial and ongoing assessments of LTC and TCU patients.

  • Develop and implement individualized care plans based on medical history and patient needs.

  • Provide routine and urgent medical care, including diagnosing and treating acute and chronic conditions.

  • Prescribe medications, order diagnostic tests, and adjust treatment plans accordingly.

  • Collaborate with an interdisciplinary team (nurses, therapists, social workers) to optimize patient care.

  • Educate patients, families, and caregivers on treatment options, medication adherence, and disease management.

  • Ensure documentation is accurate and compliant with regulatory standards.

  • Address end-of-life care decisions and collaborate with palliative and hospice teams when needed.

Qualifications:

  • MD or DO degree from an accredited medical school.

  • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred.

  • Experience in long-term care, transitional care, or geriatrics is preferred but not required.

  • New Graduates are Accepted!!
  • Valid medical license to practice in Wisconsin.
