Job description

Home Health Licensed Practical Nurse (LPN)


This is a PRN position, which means you will work on an as-needed basis. Your schedule will vary depending on the cases available and the number of hours required for each case.


Responsibilities: 

  • Provide in house care to patients with illnesses, injuries and chronic diseasesโ€”often after hospital dischargeโ€”under the supervision of an RN
  • Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
  • Take patient vitals, do assessments, document progress and report changes to RN supervisor
  • Assist RN in carrying out patientโ€™s plan of care and teaching patient self-care techniques
  • Educate family on the patientโ€™s plan of care, medication and home safety  
  • Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

Requirements:

  • Graduate of an accredited practical nursing program and active LPN license in GA
  • Experience in home healthcare preferred 
  • Negative TB test (within past 12 months)
  • Current CPR certification 
  • Valid driver's license with auto insurance 
  • Able to submit weekly care logs/time sheets to our Augusta office 
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills and compassion
