Job description

Westlake Nursing and Rehabilitation Center is seeking compassionate, dependable, and dedicated Licensed Practical Nurses (LPNs) to join our nursing team. If you are passionate about providing exceptional care to seniors and enjoy working in a collaborative environment, we encourage you to apply.

Location: Westlake Nursing and Rehabilitation Center
Position: Licensed Practical Nurse (LPN)
Shift: 7:00 AM – 3:00 PM | 3:00 PM – 11:00 PM | 11:00 PM – 7:00 AM (Weekend and holiday availability may be required)


Duties and Responsibilities

  • Administer medications, treatments, and therapies as prescribed.

  • Monitor and assess residents, reporting changes in condition to the RN and physician.

  • Supervise and support Certified Nursing Assistants (CNAs).

  • Maintain accurate and timely documentation in the electronic medical record (EMR).

  • Assist with admissions, transfers, and discharges.

  • Collaborate with physicians, therapists, social services, and other interdisciplinary team members.

  • Ensure compliance with CMS, state, OSHA, HIPAA, and facility policies.

  • Promote resident dignity, safety, and quality of life.

  • Respond promptly to resident needs and emergency situations.

  • Participate in care planning meetings, in-service education, and quality improvement initiatives.

    Qualifications
    • Current, active Licensed Practical Nurse (LPN) license in the State of Florida.

    • Graduate of an accredited Practical Nursing program.

    • Current CPR/BLS certification.

    • Minimum of One-Year Long-term care or skilled nursing experience required.

    • Strong communication, organizational, and critical thinking skills.

    • Ability to work independently and as part of a multidisciplinary team.

    • Experience with electronic medical records (EMR) is required.
