Overview
Reporting to the AVP of Nursing Operations and Post-Acute Care, the Director, Post-Acute Care is responsible for ensuring patients served by UF Health Jacksonville transition seamlessly from inpatient care to the most appropriate post-acute setting in a safe, high-quality environment that meets their individual needs.
The Director provides strategic and operational leadership across Post-Acute Care Services, driving exceptional patient safety, quality, and experience while promoting continuous process improvement. This role is focused on ensuring patients receive the right care in the right setting through cost-effective, coordinated care delivery.
In addition to oversight of acute care case management services, the Director is responsible for the leadership and management of all Post-Acute Care business units, fostering strong internal and external partnerships with post-acute providers and community organizations that support the continuum of care. Serving as the hospital's primary liaison for post-acute partners, the Director monitors partner performance and ensures desired clinical, operational, and patient outcomes are achieved.

    • Transitional Care Unit (TCU)
    • Case Management
    • Palliative Care
    • Utilization Management
    • Home Health
    • Hospital at Home
    • Transition of Care team
  • Integrate the mission, vision, and values of UF Health Jacksonville across all Post-Acute Care Services.
  • Provide strategic leadership and direction for Post-Acute Care Services and facilities, ensuring the resources necessary to deliver high-quality patient care.
  • Foster strong clinical and operational partnerships between UF Health Jacksonville and community partners that impact patient care.
  • Develop strategic internal and external relationships that improve access, capacity, quality, service, and affordability across the continuum of care.
  • Recruit, motivate, develop, and retain a diverse, high-performing workforce while establishing effective recruitment, retention, performance evaluation, promotion, and competency standards.
  • Share accountability for the quality, safety, integration, and operational efficiency of clinical services throughout Post-Acute Care Services.
  • Lead continuous quality improvement initiatives that enhance patient care, clinical outcomes, and coordination of services while ensuring care delivery aligns with patient acuity, community needs, staff competencies, available resources, and established healthcare benchmarks.
  • Define, develop, and implement strategic goals, programs, and performance metrics for patient and support services.
  • Actively participate in resolving patient concerns and complaints to promote the highest level of service excellence.
  • Champion programs that promote patient safety and clinical quality across all Post-Acute Care services and facilities.
  • Develop and manage departmental budgets while implementing strategies that effectively manage costs and improve quality of care.
  • Assist with payor relationships and oversee utilization management initiatives and strategies.
  • Ensure compliance with all applicable regulatory and accrediting bodies affecting Post-Acute Care Services and facilities.
  • Serve as the hospital's primary contact for post-acute partners, monitoring performance and ensuring expected outcomes are achieved.

Qualifications
Education:
  • Master's degree required.
  • Master's degree in nursing, Public Health, Healthcare Administration, or a related field preferred.

Licensure & Certifications:
  • Current Florida Registered Nurse (RN) license required.
  • Florida Nursing Home Administrator license preferred.

Experience:
  • Minimum of 7 years of clinical care experience in a complex academic medical center required.
  • 5-7 years of progressive healthcare leadership experience required.
