Job description

Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking a Population Health Analyst for the Rural Health Transformation (RHT) Program.

The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.

Primary Responsibilities:

  • Under the direction of the Director, Population Health, and in collaboration with the Population Health, Clinical and IT teams, the Population Health Analyst is responsible for reporting and analytics support on assigned projects including provider performance analysis, performance targets, quality measure performance, intervention outcomes, alternative payment models and other contractual deliverables.
  • In addition, the Population Health Analyst will support and promote the use of the predictive modeling tool, CareAnalyzer®, including monthly maintenance and validation, application updates, as well as reporting and ad-hoc analysis.

Tasks Performed:

  • Performs all aspects of data analyses (data acquisition, data management, documentation, visualization of results, evaluation and written interpretation of the data, and validation/testing); identifies data issues and performs research, design and analysis for creation of standard and ad-hoc reports.
  • Develops and implements advanced analytic and data mining methods.
  • Assists the Population Health team with monthly CareAnalyzer® maintenance and validation, application updates, and integrates CareAnalyzer® predictive modeling data into ad-hoc and routine reports.
  • Performs required analytics to support DSS special projects and grants (e.g. Maternity Bundle, AHEAD, Rural Health Transformation, etc.).
  • Supports Population Health Analytics team in developing alternative payment models including collaborating with external vendors and stakeholders, reporting and analysis of applicable key metrics, and other ad-hoc reporting as needed.
  • Collaborates with the Health Services, Provider Engagement Services and IT teams to improve the utility and user friendliness of customer facing applications, analysis and reporting.

Essential Functions:

  • Conducts research and develops SQL and other programming routines to perform custom analyses of moderate to complex healthcare data, supporting program improvement initiatives.
  • Develops and provides summaries and ad-hoc reports for both internal and external clients.
  • In collaboration with the Director, Population Health analyzes performance results and delivers reports and profiles to customers related to the following: medical management; care management programs; disease management programs; clinical quality; member outreach; provider quality; and utilization management.
  • Provides analytic support for the Maternity Bundle, AHEAD, Rural Health Transformation, and other alternative payment models and projects.

Desired Education: Two to four years post-secondary schooling

Desired Degree: Bachelor's degree required, Master's degree preferred

Desired Major: Business or health care-related discipline

Desired Job Experience: Minimum of one to three years of experience in health outcomes reporting and analysis, health economics, actuarial/underwriting, healthcare finance, or other related health plan reporting and analytics role required.

Other Qualifications: Familiarity with predictive modeling applications and HEDIS quality measures. Experience with report writing tools and visualization applications including SQL, Tableau and Qlik Analytics (working experience in dashboard generation). Advanced working knowledge of Microsoft Office products specifically Access and Excel.
