Family Nurse Practitioner (FNP) or Physician Assistant (PA) – Primary Care | Chicago, Illinois About the Opportunity

A well-established, mission-driven Federally Qualified Health Center (FQHC) serving the Chicago area is seeking a bilingual Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join its primary care team.

Polish language fluency is absolutely required. This position serves a large Polish-speaking patient population, and candidates must be fluent in both English and Polish. Unfortunately, candidates who do not speak Polish cannot be considered.

This respected community health organization has provided comprehensive, affordable healthcare to underserved communities throughout the Chicago area for decades. Offering integrated medical, dental, behavioral health, women's health, pediatrics, and pharmacy services, the organization is committed to improving access to high-quality care while making a meaningful impact on the communities it serves.

The position is based in Chicago's Belmont Cragin neighborhood, a diverse and vibrant northwest-side community with convenient access to downtown Chicago, excellent dining, parks, and cultural attractions.

Position Highlights

Clinic Hours
  • Monday: 6:30 AM – 8:00 PM
  • Tuesday: 8:30 AM – 5:00 PM
  • Wednesday: 6:30 AM – 8:00 PM
  • Thursday: 7:30 AM – 8:00 PM
  • Friday: 8:30 AM – 5:00 PM
  • Saturday: 8:00 AM – 2:00 PM
Qualifications
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Board Certified with certification maintained in good standing
  • Active or eligible Illinois APRN or PA license
  • Illinois Controlled Substance License and DEA registration
  • Current provider-level CPR certification
  • Fluent in both Polish and English (required)
Compensation & Benefits
  • Competitive salary: $97,000–$130,000, based on experience
  • Full Practice Authority NPs start at a higher salary scale
  • $20,000 salary increase after one year for eligible providers meeting productivity/template requirements
  • Eligible for NHSC and Illinois State Loan Repayment Programs
  • 27 days PTO annually
  • 3 personal days
  • 1 floating holiday

  • $3,000 CME allowance plus 5 CME days
