JOB SUMMARY: Under the direction of Director of Nursing or Assistant Director of Nursing. Provides nursing care to residents in the Health Care Center through assessment, planning implementations and reevaluation of resident needs. Carries out and interprets policies, procedures, local, state, and federal regulations in the Health Care Center. Responsible for directing care given by Certified Nurse Assistants.

  • Assesses resident needs, and provides necessary nursing care as per resident's care plan.
  • Completes assigned MDS and participates in interdisciplinary care plan conferences.
  • Assures that resident rights are maintained.
  • Provides emotional and spiritual support to residents, families, and other staff as needed.
  • Administers prescribed medications and treatments in accordance with approved nursing techniques.
  • Supports a restorative philosophy of care.
  • Works to maintain a clean, safe, homelike atmosphere, and respects the individuality of each resident.
  • Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures.
  • Maintain regular and punctual attendance at work and meetings.
  • Attends in-service and education programs.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

  • Understanding of Point Click Care or similar systems
  • Experience in long term care or assisted living recommended
  • Knowledge of monitoring medication
  • Excellent communication and interpersonal skills
  • Ability to work independently.
  • Proficient with Microsoft Office Suite or related software.
  • Solid organizational skills including attention to detail and multi-tasking skills.
  • Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, families, and employees.


Requirements

  • Current Licensed Practical Nurse (LPN) License in the State of Illinois.
  • Minimum two years' experience in similar environment preferred.
  • Excellent interpersonal, written and verbal communication skills.
  • Ability to work efficiently under time constraints.
  • Strong leadership skills and ability to work collaboratively with a large team
