WHO WE ARE LOOKING FOR:
Franciscan Physician Network is seeking an Advanced Practice Provider to meet the growing demand for Physiatry (PM&R) services at Franciscan Health Dyer located in Dyer, IN with outreach to include Crown Point.
WHAT WE NEED:
Candidate will help manage inpatient rehab floor inside Dyer hospital under a collaborating PM&R physician to coordinate and deliver the highest quality of patient care. Candidate will need to be a day-to-day medical manager for medically active rehab patients to ensure they remain stable enough to stay engaged in intensive rehab.
WHAT YOU CAN EXPECT:

  • Hours: Monday - Friday, 8:00 a.m. - 4:30 p.m.
    • Every 4th weekend at Dyer and Crown Point
      • Both campus on one day (Sat or Sun) or 1 campus each day
  • Average inpatient volume: 10-15 patients per day
  • Will need to do H&P's, progress notes, discharge summaries, and medication reconciliations on discharge
  • Daily follow up on medical issues like blood pressure, Diabetes, pain control, bowel/bladder management, infections
  • Evaluate new symptoms or clinical changes so therapy can continue safely
  • Handle routine orders including lab work, medication adjustment, and imaging
  • Coordinate patient care with therapy, nursing, case management, consultants, and attending physician
  • Assistance with consults may also be needed depending on the day
