Job description

As a Behavioral Health Consultant for Swope Health Services Wyandotte Clinic , you will be apart of a team of caregivers that provide care for the people that need it the most. As Behavioral Health Consultant(BHC) you will provide brief, targeted, real-time assessments and/or interventions during office visits to address the psychosocial aspects of clients in primary care and to follow-up on all referrals from primary care providers. The BHC is an embedded, full-time member of the Primary Care Team and will collaborate with other Swope Health Services department and will refer clients for care.

Primary Functions: 

  • Completes initial brief psychosocial assessments for children, youth, and adults in Primary Care.
  • Provides assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders.
  • Develops and implements, in collaboration with the Primary Care Team, an integrated, brief treatment plan and assists clients in successfully achieving goals.
  • Engages individuals and families as partners in services and develops positive, hopeful, and trusting relationships.
  • Monitors, evaluates, and records client progress with respect to treatment goals and modifies treatment plans according to changes in client status.
  • Evaluates crisis situations and applies appropriate interventions.
  • Actively participates in interdisciplinary treatment team meetings that support an integrated health care model to provide comprehensive care for clients. The BHC will provide clinical support and consultation to team members toward advancement of consumer treatment plan goals.

Ideal Candidate Will Have

  • Licensed Clinical Social Worker (LCSW), Licensed Clinical Psychologist (LCP), or a PhD in Clinical Psychology is preferred.
  • A Licensed Professional Counselor (LPC), Licensed Master’s Social Worker (LMSW), Provisionally Licensed Professional Counselor (PLPC), or other Master’s Level Clinicians in the Behavioral Health Field, preferably seeking licensure. 
  • One to Three years of professional experience working with seriously and persistently mentally ill adults. Experience preferred with clients who have experienced trauma, homelessness, and/or substance abuse; prefer one year in a community mental health setting.
  • Proficient knowledge, utilization, and/or familiarity of the DSM-5/ICD-10, ability to assess mental health disorders, and make person centered treatment and service recommendations.
  • Proficient knowledge of health and human services and community resources.
