As a Home Health Physical Therapist Assistant, hereโ€™s a big-picture view of what youโ€™ll do:

  • Provide home-based care under the supervision of a Licensed Physical Therapist to patients with illnesses, injuries and chronic diseasesโ€”often after discharge from a hospital, rehab or nursing facility
  • Work with a team of physicians and oversee paraprofessional staffย providing patient care
  • Examine patients, complete assessments, document progress and report changes to their physician
  • Educate patients on their plan of care, prescribed medication, therapy, diet and exercise

  • HCHB charting experience
