Job description

Clinical Pharmacy Specialist (Informatics Program Manager)

The Clinical Pharmacy Specialist (Informatics Program Manager) includes but is not limited to:

  • Demonstrates the knowledge and ability to apply systems redesign principles to improve processes and identify novel approaches in data collection, management and analyses.
  • Develop informatic tools that will assist in meeting clinical performance goals with respect to medication management. Facilitate collection of accurate, detailed and useable data relevant to the screening and assessing the patient's medication regimen for clinical effectiveness and therapeutic outcomes.
  • Responsible for the development, maintenance, and continuous improvement of informatics management, including those involving highly innovative methods of health care delivery in the VA Loma Linda Healthcare System.
  • Performs workflow assessment and optimization in clinical, administrative, educational, and research domains, in addition to adapting software controls to existing workflow.
  • Assists in the operation of other areas of Pharmacy Service when required, including distribution and clinical work in other areas (e.g., medication order review and verification).
  • Provides physicians and other providers with a focal resource for discussion of medical problems requiring pharmacy informatics intervention.
  • Actively participates in the formal and informal education of pharmacy trainees when requested by the Service to serve as a preceptor for students and/or post-graduate pharmacy residents. Minimally, this involves planning, supervising, and implementing graduate, undergraduate, residency, and internship programs offered in cooperation with affiliated institutions.
  • Responsible for the overall pharmacy informatics related management of the designated pharmacy clinic/clinical area, including but not limited to developing and maintaining clinic grids, sustaining clinic utilization, managing missed opportunities, coordinating clinic cancellations, managing discharge planning, reducing medication-related rehospitalization, and restructuring as necessary to effectively maintain patient care in response to staffing needs.
