Job description

The LPN provides direct primary nursing care to residents and supervises the care provided by certified nursing assistants.

ESSENTIAL FUNCTIONS:

  1. Keeps administrative nursing staff informed on personnel development.
  2. Provides feedback to Nurse Managers regarding performance of nursing assistance.
  3. Participates in committees as assigned.
  4. Demonstrates ability to address service concerns and follow up timely for resolution.
  5. Supports all efforts through the facilityโ€™s Service Excellence Initiative.
  6. Effectively supervises and directs licensed and non-licensed nursing personnel.
  7. Responsible for developing CNAโ€™s resident care assignment and other duties as needed, prior to the beginning of each shift.
  8. Promotes and maintains positive and collaborative working relationship with physicians, physician-extenders (NPs or PAs), lab, diagnostic personnel, and pharmacy to ensure proper and safe care of residents.
  9. Promotes and maintains positive and collaborative working relationship with nursing personnel, other departments, residents, families, and visitors.
  10. Observes residents daily. Reports any significant change of resident's condition to the physician. Completes routine nursing documentation for assigned resident.
  11. Monitors to ensure that nursing care documentation is accurate, concise, and timely.
  12. Administers and documents medications using the five rights of medication administration, while adhering to the principles of infection prevention and control practices.
  13. Provides and/or receives shift report specific to changes or concerns regarding residents to oncoming and off going shift.
  14. Implements pharmacy processes regarding ordering/reordering of medications, utilizing STAT/Emergency medication inventory when indicated, receiving, and returning medications.
  15. Performs physical assessment for residents, recognizes normal and abnormal physiological findings and based on the residentโ€™s baseline. Reports findings as indicated.
  16. Performs wound care in accordance with physician/physician-extender orders. Documents treatments completed.
  17. Assists in the development of the residentsโ€™ plan of care. Resident care is provided in accordance with the plan of care.
  18. Assists with updating the residentโ€™s plan of care as indicated.
  19. Keeps administrative nursing staff informed regarding evaluations of resident care.
  20. May participate in rounds with physicians, physician-extenders, other healthcare personnel to provide pertinent information regarding the residentโ€™s care/condition and follows through with the physicians/physician-extender orders.
  21. Reviews physicianโ€™s/physician-extender orders, lab/diagnostic reports, and contacts pharmacy regarding any change in medication.
  22. Effectively communicates with family members regarding resident's care and/or condition.
  23. Completes accident/incident report and do follow up. Will report all accidents/incidents to administrative nursing staff. Notifies physician and family.
  24. Performs routine nursing skills competently and professionally to include but it not limited to:ย 
    • Phlebotomy
    • Initiation of peripheral intravenous (IV) therapy/Care of Central lines
    • Wound care.
    • Medication administration
    • Insertion of urinary catheters
    • Administration and management of tube feeding formula.
    • Oxygen administration
    • Tracheostomy care
    • Airway management
    • Physical assessment
    • Skin assessments
  25. Communicates staffing problems and needs to unit manager/supervisor.
  26. Reports service and unit concerns to the unit manager/supervisor.
  27. Receives and provides end of shift reports.
  28. Assists in admission, transfer, and discharge of residents.
  29. Reports maintenance and housekeeping needs accordingly.
  30. Maintains narcotic records accurately. Validates narcotic/scheduled medications before shift starts and before leaving for the day/shift. Notifies unit manager of discrepancies in medication inventories.
  31. Performs other duties, as assigned.
  1. Current LPN license in good standing with the Virginia Department of Health Professions.
  2. Two years of experience as a LPN, preferred.
  3. Demonstrates excellent clinical, customer service and leadership skills.
  4. Current CPR Healthcare Provider certification.

PHYSICAL REQUIREMENTS:

  • Prolonged periods standing, walking, working and frequently pulling, pushing, and bending.
  • Must be able to lift 25 pounds at a time.
  • Must be able to move 150 pounds with assistance.
