Job description

Are you a dedicated Clinical Pharmacist who is passionate about making a meaningful impact in the healthcare industry? Then we invite you to join our growing dynamic team at Axil Health! We are seeking an exceptional Pharmacist to lead our Chronic Care Management (CCM), Transition Care Management (TCM), and Remote Patient Monitoring (RPM) services. This full clinical service program will operate through a Collaborative Practice Agreement with independent medical facilities across North Carolina, South Carolina and Virgina.

As a Clinical Pharmacist, you will establish and maintain professional relationships with all team members within the medical practice. Responsibilities may vary depending on the unique needs of each independent practice, offering you a diverse and stimulating workload. Must live in a commutable distance to the posted job location โ€“ this role is performed in provider clinics and may require regional travel to a group of clinics.


In this role, you will have the opportunity to:

  • Build and work directly with your patient panel to drive clinical outcomes and results.
  • Spearhead patient enrollment, device management, and data interpretation for RPM.
  • Collaborate on medication management with supervising providers.
  • Participate and lead topic discussions for current staff and residents.
  • Precept PGY-1 Students and participate in our Fellowship Program.
  • Develop individualize care plans based on specific patient needs.
  • Provide recommendations or implement therapy changes, when necessary.
Requirements Must Haves:
  • A current, valid pharmacist license to practice in North Carolina.
  • Current CPR certification.
  • Certification for Immunization.

Please note, this career opportunity is open to all experience levels. Retail pharmacists and current clinical pharmacists are all encouraged to apply through preferred candidates would also have one of the following:

  • PharmD Degree with three years of experience.
  • Currently, hold a Board of Pharmaceutical Specialty Certification or Completion of APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program.
  • Completion of an APhA Certificate Training Program.
