Summary: 

The Ambulatory Care Pharmacist practices within physician offices within Scotlands Health Care System to provide protocol-driven, evidence-based patient care.

Job Duties: 

  1. Educates and consults with patients and providers regarding the use of medications, disease management, potential drug interactions, and monitoring of drug therapies.
  2. Provides team-based care, including working with multidisciplinary teams, and care management may be involved.
  3. Responds to requests for drug information from healthcare providers and patients.
  4. Evaluates the usefulness of biomedical literature gathered and formulates responses to drug information requests based on analysis of the literature.
  5. Provides medication-use education and counsels patients and caregivers.
  6. Performs co-visits with patients and prescribes medications as appropriate.
