Menomonee Falls, WI
Pay From: $61.00 per hour

MUST:
Experienced Pharmacist
5 plus years of experience as a Pharmacist
Must have an active Wisconsin Pharmacist license
Must have inpatient experience
Must have PIC (Pharmacist in Charge) experience
DUTIES:
Pharmacist will be responsible for, but not limited to the following:
Review all patient charts for correct medication therapy and documentation; including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and dose
Conduct weekly on-site visits to each facility
Conduct monthly unit review of department
Assist Hospital Administrator to ensure sufficient medication is in stock; including working with the pharmaceutical wholesaler to find alternatives, if necessary
Review and reconcile controlled substances logs and conduct an annual controlled substance inventory
Assist clinical staff with drug related information
Counsel patients on medication therapy, if needed
Plan, monitor and evaluate drug programs and regimens
