Job description

JOB SUMMARY

A licensed Pharmacist is responsible for ensuring that medications and IVโ€™s are prepared, dispensed, and delivered to nursing service in an accurate and timely manner. The Pharmacist receives orders electronically, written, phoned, or faxed by credentialed medical staff and enters/verifies them into the patient profile via the computer. They prepare or supervise the preparation of all IVโ€™s and IV admixtures by a Pharmacy Technician. It is the Pharmacistโ€™s responsibility to check all work done by a Pharmacy Technician before it is delivered to a nursing area. Education is provided to patients, nurses, medical staff, and any other group that requests information on medication. The Pharmacist is responsible for supervising all Pharmacy Technician and Pharmacy Assistant activity within the Pharmacy.

PRINCIPLE FUNCTIONS include the following, other duties may be assigned:

  • Dispenses or supervises the dispensing of all medication throughout the hospital utilizing a modified unit dose system.

  • Maintains a working inventory of all medications. Populates orders with the wholesaler and/or manufacturers as needed.

  • Receives and interprets physician orders then enters orders into patient profile in computer noting patient allergies. Clinically addresses each order for age-specific dosing, checks for drug interactions, and if appropriate, assures via Lab tests (MICโ€™s) that the indicated antibiotic is ordered. When necessary, confers with physician re: changes in medication, dosage, schedule, etc.

  • Therapeutically substitutes medications as needed per policy

  • Adjusts medications as needed per policy for renal and hepatic function

  • Adjusts vancomycin, aminoglycosides, warfarin, and direct oral anticoagulant therapy as needed per policy

  • Oversees the medication history process for inpatients as well as outpatients as requested

  • Records and investigates Adverse Drug Reaction Reports.

  • Verifies all work of Pharmacy Technicians, Pharmacy Assistants, and Pharmacy Interns/Externs.

  • Participates in education of patients, nursing staff, medical staff and the public when appropriate.

  • Researches and responds in a timely fashion to all questions from health care workers.

  • Interacts with patients and families, nurses, physicians, and other hospital staff and detailers from various manufacturers.

  • Participates in continuing education programs as necessary to maintain license and enhance professional knowledge pertinent to the department needs.

  • Aids in the training of new pharmacists and pharmacy technicians.

  • Performs additional duties as assigned by the Director of Pharmacy.

JOB KNOWLEDGE & QUALIFICATIONS

Education

  • Bachelor of Pharmacy or PharmD from an accredited college of pharmacy required.

Training and Experience

  • Two years of hospital experience preferred.

  • Must communicate effectively in both oral and written format.

  • Must organize and plan work efficiently, demonstrate good judgment and supervisory skills.

  • Must work well with other people in a โ€œteamโ€ environment.

    Certificates, Licenses, Registrations

  • Washington State DOH Pharmacist License required.

  • Current BLS HCP required.
