Job description

Under limited supervision, provides clinical pharmacy servicesincluding primary care services, consultative services, and drug informationservices to patients and health care providers to deliver high qualitycare. Provides education and training tomedical and pharmacy students and residents. Ensures accuracy of work completed by support personnel, in compliancewith established policies and procedures.
Responsibilities:
• Reviewsprescriptions as written by authorized prescribers to determine that order issafe and appropriate for the specific patient; contacts authorized prescriberas necessary to review vague or illegible prescriptions.
• Fillsprescriptions as ordered by authorized prescriber by dispensing or compoundingmedication according to established procedures.
• Interviewspatients for previous medication history and provides counseling on currentdrug therapy. Identifies patients whorequire teaching, and provides educational material. Also works collaboratively with other healthcare providers to assure coordination of the provision of patient education.
• Assigns,educates, and supervises the work of support personnel (technicians, interns,clerks and students) in the preparation of medications for dispensing and inrelated duties.
• Ensuresmaintenance of an inventory of medication and pharmacy supplies and relatedrecords. Ensures that storage ofmedications and supplies is in compliance with environmental and regulatoryagency procedures.
• Bachelorof Science in Pharmacy or Doctorate of Pharmacy from an accredited College ofPharmacy or completion of foreign equivalency examination.
• Current Michigan RPh license or Michiganeligibility along with required Controlled Substance license.
