Chesapeake Regional Pharmacy Services was founded in Wilmington, DE by pharmacists on the principle of providing patient-centric, personal, and specialized care to patients needing long term care pharmacy in the Delaware region and surrounding cities.
Position Summary
At Chesapeake Regional Pharmacy Services, we take pride in serving individuals in Long-Term Care (LTC) and residential care settings with compassion, accuracy, and integrity. We are seeking a dedicated LTC Pharmacist who shares our commitment to exceptional patient care-someone who ensures every resident receives the right medication, at the right dose, at the right time.
About the role
We are hiring a Pharmacist to join our growing closed-door pharmacy located in Wilmington, Delaware. As an LTC Pharmacist, you will play a vital role in ensuring safe, accurate, and timely medication management for residents in Long Term Care Facilities.
This pharmacist will be an integral member of the pharmacy team. Your success will be driven by your ability to lead and foster a culture of continuous improvement, quality, and service. The most successful pharmacists demonstrate excellent customer service skills, as well as the ability to prioritize the workflow.
What you'll do

  • Maintain security of the pharmacy
  • Adhere to required delivery schedules
  • Knowledge of DEA and state law with regards to controlled drug dispensing.
  • Follow all applicable state and government regulations including HIPAA.
Medication Verification & Clinical Review
  • Verify orders entered by technician staff for accuracy.
  • Conduct DURs, allergy checks, drug interactions, and therapeutic duplication reviews.
  • Review MARs, provider notes, facility orders, and eMAR entries for clarity and compliance.
  • Ensure accurate entry of orders
  • Check for completeness and precision of all new orders.
  • Check for content and accuracy of all drug packaging and labeling.
Clinical & Patient-Centered Care
  • Provide clinical recommendations to providers, nurses, and caregivers.
  • Review PRN and routine medications for appropriateness and resident safety.
  • Support high-risk medication monitoring (e.g., anticoagulants, insulin, psychiatric medications).
  • Assist with Consulting Pharmacist duties, including Medication Regimen Reviews (MRR).
  • Participate in care plan meetings, audits, and quality improvement initiatives.
Regulatory & Compliance
  • Assist in maintaining a perpetual inventory for schedule II drugs.
  • Knowledge of DEA and state law with regards to controlled drug dispensing.
  • Follow all applicable state and government regulations including HIPAA.
Qualifications
Required
  • Doctor of Pharmacy (PharmD)
  • Active Delaware Pharmacist License (RPh or PharmD)
  • Strong communication skills with nurses, facilities, providers, and internal teams
  • Ability to work efficiently in a fast-paced LTC workflow with exceptional attention to detail
  • Experience using pharmacy management systems

Location
This is an on-site position located in Wilmington, Delaware (New Castle County).
We serve facilities across New Castle County, Newark, Wilmington, Dover, and surrounding areas .
