About the Legacy Lyfe Pharmacies
Legacy Lyfe Pharmacies is a growing 503A sterile compounding pharmacy group focused on quality, safety, and responsible multi-state expansion. We are building a sterile-heavy, inspection-ready operation, located in Basalt, Colorado, designed to scale while maintaining the highest regulatory and clinical standards.
About the Role
The Basalt Pharmacy location is seeking an experienced Pharmacist-in-Charge (PIC) to lead sterile compounding operations, quality assurance, and multi-state regulatory compliance. This is a senior, hands-on leadership role for a pharmacist who is comfortable owning inspection outcomes, sterile risk, and growth-stage complexity. This role combines statutory PIC responsibility with hands-on QA leadership and pharmacy management in a high-throughput sterile environment.
The ideal candidate brings deep experience with USP lt;797 gt;/ lt;800 gt;, thrives in inspection-ready operations, and is excited to help scale a growing compounding pharmacy across multiple states.
This is not a retail, hospital-only, remote, or paper-PIC position.
Key Responsibilities
PIC amp; Regulatory Leadership
  • Serve as licensed Pharmacist-in-Charge in accordance with state and federal regulations
  • Ensure compliance with state and federal pharmacy laws, DEA requirements, and USP lt;795 gt;, lt;797 gt;, lt;800 gt;
  • Act as primary regulatory contact, leading regulatory inspections, audits and accreditation surveys (resident and nonresident states)
  • Maintain pharmacy, DEA, and multi-state licensure
  • Ensure continuous inspection readiness and timely remediation of findings
Quality Assurance amp; Sterile Compounding
  • Own and maintain the pharmacy Quality Management System (QMS)
  • Oversee sterile compounding compliance under USP lt;797 gt; and lt;800 gt;, including:
    • Environmental monitoring (viable and non-viable)
    • Media fills and personnel qualification/requalification
    • Aseptic technique, garbing, cleaning, and disinfection programs
  • Oversee compounding of sterile preparations from sterile and non-sterile starting components, including appropriate sterilization methods, validation, and documentation
  • Review and approve:
    • Master Formulation Records (MFRs)
    • Compounding Records (CRs)
    • Deviations, investigations, and CAPAs
  • Maintain inspection-ready documentation and data integrity
Pharmacy Operations amp; Growth
  • Lead day-to-day sterile and non-sterile operations
  • Train, mentor, and supervise pharmacists, technicians, and support staff
  • Align staffing, workflows, and cleanroom capacity with volume growth
  • Support multi-state expansion, nonresident licensing readiness, and operational scale-up
  • Partner with leadership on hiring, performance management, and continuous improvement
Statutory Accountability
The Pharmacist-in-Charge (PIC) shall have full responsibility for the professional practice of pharmacy at Legacy Lyfe Pharmacies and shall be accountable for compliance with all applicable:
  • State and federal pharmacy laws and regulations
  • Board of Pharmacy rules
  • DEA regulations
  • USP lt;795 gt;, lt;797 gt;, lt;800 gt;, and other applicable USP chapters
The PIC is responsible for ensuring that pharmacy operations are conducted in a manner that protects patient safety, maintains drug integrity, and meets all regulatory requirements.
Regulatory Oversight, Inspections amp; Accreditation
The PIC shall:
  • Serve as the primary regulatory contact for:
    • State Boards of Pharmacy (resident and non-resident)
    • DEA and other regulatory agencies
    • Accreditation bodies, as applicable (e.g., PCAB or equivalent)
  • Ensure all pharmacy licenses, permits, registrations, and renewals are current and accurate
  • Notify applicable Boards of Pharmacy of reportable changes, incidents, or PIC transitions in accordance with state law
  • Ensure inspection readiness at all times and oversee responses to regulatory findings, citations, or corrective actions
Personnel amp; Supervision
The PIC shall:
  • Ensure appropriate supervision of all pharmacists, technicians, interns, and support personnel
  • Verify that all pharmacy staff:
    • Are properly licensed, registered, or credentialed
    • Are trained and competent for assigned duties
  • Ensure adequate staffing levels to safely support sterile and non-sterile compounding operations
Compounding amp; Quality Assurance Oversight
The PIC shall be responsible for oversight of all compounding activities, including:
  • Approval and maintenance of:
    • Policies and procedures
    • Master Formulation Records (MFRs)
    • Compounding Records (CRs)
  • Oversight of sterile compounding compliance, including:
    • Environmental monitoring programs
    • Personnel qualification and requalification
    • Aseptic technique, garbing, cleaning, and disinfection
  • Review and approval of deviations, investigations, and corrective and preventive actions (CAPAs)
  • Ensuring appropriate documentation, record retention, and data integrity
Nonresident Pharmacy Licensing Oversight
The Pharmacist-in-Charge (PIC) shall have responsibility for oversight of resident and nonresident pharmacy compliance, including but not limited to:
  • Supporting and maintaining nonresident pharmacy licensure in states where Legacy Lyfe Pharmacies dispenses compounded preparations
  • Ensuring compliance with:
    • State-specific nonresident pharmacy requirements
    • Pharmacist-in-Charge designation requirements, where applicable
    • Inspection, reporting, and renewal obligations
  • Collaborating with internal teams and external partners to:
    • Prepare applications, renewals, and required documentation
    • Respond to Board inquiries, audits, or inspections related to nonresident licensure
  • Ensuring pharmacy operations, policies, and documentation remain aligned with multi-state regulatory expectations
While administrative aspects of licensing may be delegated, the PIC retains ultimate responsibility for ensuring compliance with all applicable nonresident pharmacy laws and regulations.
Delegation
While certain operational or administrative tasks may be delegated, the PIC retains ultimate responsibility and accountability for pharmacy compliance and professional practice as required by law.

Required Qualifications
  • Active, unrestricted Pharmacist license in Colorado (or ability to obtain)
  • Eligible and willing to serve as Pharmacist-in-Charge
  • 3–5+ years’ experience in a 503A compounding pharmacy
  • Strong working knowledge of USP lt;797 gt; and lt;800 gt;
  • Direct experience with sterile compounding operations
  • Hands-on involvement in regulatory inspections (Board of Pharmacy, DEA, or accreditation)
Preferred Qualifications
  • Prior PIC experience in a sterile compounding pharmacy
  • Experience with PCAB or similar accreditation programs
  • Experience supporting multi-state (nonresident) pharmacy operations
  • Experience compounding sterile preparations from non-sterile components, including sterilization requirements
  • Comfortable operating in a growth-stage or startup environment
Candidates with exclusively retail or hospital IV admixture backgrounds should be prepared to clearly demonstrate experience with non-sterile-to-sterile compounding and sterilization requirements.
Base Salary:
  • $165,000 – $195,000, commensurate with experience and PIC background
In addition to base compensation, this role offers eligibility for sign-on incentives, retention bonuses, and annual performance-based awards, aligned with organizational goals and individual contribution.
