Job description

Per Diem Inpatient Pharmacist

Pharmacy Division
The Pharmacy Division is dedicated to ensuring safe, effective, and cost-conscious medication use while collaborating with healthcare professionals to achieve optimal drug therapy outcomes. Through innovative programs in patient care, research, and education, the division leads in delivering premier, patient-centered care. Embracing change and discovery, it leverages new technologies, maximizes resources, and fosters a culture of caring to continuously enhance the quality of pharmaceutical services.
Ideal Candidate
The ideal candidate will hold a Bachelor of Science degree in Pharmacy or a Pharm.D. degree from an accredited college of pharmacy, with a professional doctorate preferred. At least one (1) year of inpatient pharmacy experience is preferred. The candidate will demonstrate strong clinical knowledge, attention to detail, and the ability to work effectively within a multidisciplinary healthcare team.
Schedule
Working hours will be Per Diem and on an as needed basis.

Job-Specific Competencies
Core Pharmaceutical Services
Responsibilities include:
  • Reviewing, clarifying, and processing medication orders accurately and in a timely manner.
  • Assessing the appropriateness of ordered medications based on disease state, body surface area (BSA), age, height, weight, laboratory values, and renal function.
  • Utilizing computer systems and clinical resources to support pharmaceutical care, including medication order entry and verification, medication distribution and checking, and medication reconciliation.
  • Performing and documenting therapeutic interventions as appropriate.
  • Actively participating in event reporting through UHC Safety Intelligence.

Pharmacy Area Supervision, Administrative, and Systems-Based Practice
Responsibilities include:
  • Supervising immediate technical staff by ensuring assignment completion, monitoring lunch breaks, and maintaining adherence to institutional and departmental policies, including sterile and non-sterile compounding requirements.
  • Ensuring timely completion of tasks to maintain workflow and support optimal patient care.
  • Maintaining a clean and organized work area.
  • Monitoring inventory and supplies and notifying appropriate personnel of shortages or outages.
  • Overseeing sterile and non-sterile compounding activities.
  • Completing assigned Quality Assurance and productivity reports, including documentation and follow-up.
  • Researching and monitoring operational issues, initiating corrective actions, and following through to resolution.
  • Providing recommendations for process improvement.
  • Maintaining compliance with all institutional and divisional requirements, including licensure, mandatory education and testing, Infection Control, age-related competency, Code Blue, BLS certification, TB testing, and IV process validation.
  • Developing, implementing, and supporting special projects across the institution.

Clinical / Patient Care
Responsibilities include:
  • Participating in clinical initiatives such as discharge counseling, medication reconciliation, and renal dose adjustments.
  • Conducting and documenting patient education as appropriate.
  • Documenting patient-specific information in the electronic medical record.

Qualifications
Education: Bachelor of Science degree in Pharmacy or Pharm.D. degree from an accredited college of pharmacy.
Preferred Education: Doctorate (Professional)
Certification: All of the following:
  • Texas State Board of Pharmacy Pharmacist license within 90 days of job entry
  • American Heart Association (AHA) Basic Life Support (BLS) certification within 30 days of job entry
  • American Heart Association (AHA) Advanced Cardiac Life Support (ACLS) certification within 90 days of job entry if working full-time in one of the following departments: Pharmacy Inpatient (710348), Pharmacy ATC ACB (710355), Pharmacy-West Houston (710657), or Pharmacy-The Woodlands (710480)

Preferred Certification:
  • ACPE Sterile Products Certification
  • ACLS/BCLS Certification

Experience: None
Preferred Experience: One year of experience in a hospital pharmacy setting or completion of an ASHP PGY1 Residency Training Program preferred.
Work Location: Onsite presence is required.
