Job description

An immediate opportunity is available for a Staff Clinical/Infusion Pharmacist to support an 8-12 week contract in Mesa, AZ. This role is pivotal in managing and overseeing home infusion therapies, ensuring patients receive specialized, high-quality pharmaceutical care in their homes. The pharmacist will work Monday through Friday, 8:30 a.m. to 5:00 p.m., with occasional participation in an on-call rotation.
Key Responsibilities:

  • Prepare sterile specialty pharmaceuticals and intravenous (IV) solutions in compliance with state and federal regulations, maintaining a sterile and ventilated environment.
  • Monitor clinical data including lab results and pharmacokinetic dosing to optimize treatments such as vancomycin and Total Parenteral Nutrition (TPN).
  • Provide thorough patient counseling on specialty medications and treatment plans, including education on infusion pump use, sterile technique, and catheter maintenance at home.
  • Coordinate care by working closely with prescribing physicians, home health nurses, and various clinical and operational teams to ensure seamless therapy management and reduce hospital readmissions.
  • Adhere to regulatory standards including State Board of Pharmacy requirements, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
  • Manage pharmacy inventory, including refrigerated items, and ensure safe, timely packing and shipping of medications and supplies to patients.
Qualifications and Desired Experience:
  • 1-3 years of experience in infusion specialty pharmacy or hospital pharmacy IV room responsibilities.
  • IVIg experience is a plus.
  • Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
  • Current and in good standing AZ pharmacist license.
  • Experience with the CareTend system preferred.
