JF&CS is seeking a Host Home Contract Provider. - THIS IS A 1099 ASSIGNMENT.


Purpose:

The Host Home provider will act as an independent contractor who is willing to share their lives and home with an individual with intellectual/developmental disabilities. The Host Home/Life-Sharing is based on the importance of enduring and permanent relationships as the foundation for learning life skills, developing self-esteem and learning to live in interdependence with others. The experience provided will give the individual an opportunity to grow and develop to his or her greatest potential and to participate in everyday community life.


Responsibilities:

  • Provide training and support to the individuals in all daily living skills, including but not limited to nutrition, personal hygiene, health care, safety, communication, interpersonal relations, mobility, financial management, home management and use of leisure time as outlined in the Individual Service Plan (ISP).
  • Ensure individuals are taking medication according to the prescribed schedule. Assist the individual with scheduling and attending medical appointments as outlined in the Individual Support Plan. Accompany individual and facilitate participation in visits for medical care, therapies, personal shopping, recreation and other community activities as needed.
  • Provide training and support in the areas of social, emotional, physical and spiritual development.
  • Become acquainted with the individual’s life setting and communicate with key personnel supervising the individual (including job coach, support coordinator, day program, etc.).
  • Maintain a relationship with and communicate with families and key supports regarding the individual’s day to day needs and successes (if applicable).
  • Assist in the development of Individual Service Plan’s and ISP yearly summaries as requested by supervisor/JF&CS.
  • Assist in the development of Behavior Support Plan (BSP); provide behavior support as directed in the BSP.
  • Provide transportation in a privately owned car for individuals, as directed.
  • Maintain and submit all required paperwork.
  • Attend and participate in all division trainings and meetings, as required.
  • Communicate regularly with supervisor on the individual’s progress, issues and concerns.

Requirements:
  • Be at least 21 years old, with a high school diploma equivalent.
  • Be honest, dependable and caring with good judgment, understanding and patience.
  • Be a good communicator with organizational skills.
  • Be experienced in residential settings, including hands-on support with individuals who have intellectual/developmental and /or other disabilities.
  • Have a house or apartment, rented (must provide copy of lease showing 1 year or more term) or owned. The home must provide the individual(s) with a private bedroom and private or semiprivate bathroom in accordance with state regulations. The Host Home must also provide common living areas including kitchen, living room, dining room, and an outside yard/green space which the individual will share with the family.
  • Provide a written proposal (a part of the “Application to Provide Host Home Services” packet) of the physical description of the house, neighborhood, recreational facilities, transportation alternatives, restaurants, grocery stores and any other community resources that are available in the immediate vicinity of the home.
  • Successfully clear background checks, including criminal history, motor vehicle record and reference check of former employers and personal reference.
  • Complete annual training sessions, including, but not limited to: Medication Management Training, First Aide CPR, Legal Rights, Confidentiality/HIPPA, Prevention of Mistreatment, Abuse, Neglect and Exploitation, Mandatory Reporting, and Universal Precautions, Person Centered Training.
  • Possess good communication skills to communicate with IDD Director, Operational Manager, Program Manager, Nurse, Support Coordinator, Families or guardians involved in the individual’s life.
  • Carry full insurance coverage on their automobile and home (and Renter’s Insurance policy, if indicated) as well as Professional Liability insurance.
  • Be in good standing (completed documentation, responsive to requests for information, etc.) for signing a one-year, renewable contract with JF&CS.
  • Must have a Georgia State Driver’s License, Georgia registration and a privately insured vehicle; must provide proof of auto insurance that is valid for at least one year.
  • Must possess and maintain a working cell phone with voice mail or house phone with voicemail and a computer with Internet access.
  • Must possess and maintain ability to receive and respond to all business related text messaging.
  • Must have email capability and the ability to check email.
  • Must have approved site inspection of the home and no findings at monthly site visits moving forward.

Education and/or Experience:

  • High school diploma or equivalent required.
  • Associates degree preferred.
  • Experience with developmental disabilities.

Candidates who are selected for employment must successfully complete a pre-employment background screening as well as a pre-employment drug screening. Employment will be contingent upon the satisfactory completion of both the background check and the drug screening. Additionally, if applicable, tuberculosis testing (TB) and fingerprinting may also be required.
