Job description

Jewish Home Licensed Home Care is hiring Personal Care Aides to provide support services to patients in their homes in and around Monroe County
Join our growing team - $1,000 Sign on bonus!!
Pay Rate: Up to $25.00 per hour- pending experience and type of patient case
PCA or HHA certification required, and current NYS Driver's License preferred. Ensure client's safety and security by supervising the home environment. Provide assistance with activities of daily living. Perform routine housekeeping activities, provide medication reminders in accordance with authorized care plan.
EDUCATION / QUALIFICATION: New York State Certification in Personal Care Aide or Home Health Aide required, and current New York State driver's license preferred.
ESSENTIAL FUNCTIONS:
  • Ensure client's safety and security by supervising the home environment.

  • Provide assistance with activities of daily living, (grooming, dressing, bathing, walking, transferring, toileting, feeding) in accordance with authorized Care Plan.

  • Perform routine housekeeping activities such as making/ changing beds, dusting, vacuuming, washing floors, cleaning kitchen and bathroom, and laundry in accordance with authorized Care Plan.
  • Provide medication reminders per Care Plan.
  • Accurately documents services provided in client record according to policy
  • Escort clients to medical facilities, errands, shopping and outings as specified in the care plan.
  • Participate in the Care Team by providing input and making suggestions as requested.
  • Ensure service is delivered in accordance with all relevant policies, procedures and practices.
  • Monitor supplies and resources.
  • Evaluate the program and make recommendations for it, as indicated.
  • Follow the written Care Plan.
  • Observe clients and their environments and reports unsafe conditions to Supervisor.
  • Maintain client confidentiality
  • Other duties as assigned

EXPERIENCE:
  • High degree of empathy
  • Effective communication skills.
  • Has excellent writing and charting skills.
