Position Summary

Perfect Care Match is seeking an entrepreneurial, compassionate, and relationship-driven Registered Nurse (RN) Director of Community Partnerships and Growth to help expand our presence throughout Atlanta, GA and surrounding communities.

This unique leadership opportunity is ideal for a Registered Nurse who combines strong clinical credibility with exceptional relationship-building, business development, and operational leadership skills. The individual in this role will serve as a trusted ambassador for Perfect Care Match by building strategic referral partnerships, educating families and professionals about our holistic home care solutions and geriatric care management services, and supporting the organization’s long-term growth initiatives.

This role offers meaningful opportunity for leadership development and expanded operational responsibilities within a rapidly growing senior care organization.

Key Responsibilities Community Relations & Business Development

  • Build and maintain strong relationships with hospitals, physicians, discharge planners, rehabilitation centers, assisted living communities, elder law attorneys, senior centers, and healthcare referral sources

  • Represent Professional Care Match, Inc. d/b/a Perfect Care Match at networking events, educational seminars, healthcare conferences, and community outreach initiatives

  • Increase awareness of the organization’s holistic home care, geriatric care management services, wellness education and coaching throughout assigned territories

  • Develop strategic referral partnerships that support sustainable organizational growth

  • Serve as a trusted clinical and operational resource for families and healthcare professionals navigating aging-related challenges

  • Oversee Operations

Holistic Home Care & Geriatric Care Management Leadership

  • Educate families and referral partners on the benefits of personalized holistic home care services and professional geriatric care management

  • Support care coordination initiatives for seniors with complex medical, cognitive, psychosocial, and safety needs

  • Assist families with long-term care planning, advocacy, transitions of care, and aging-in-place solutions

  • Collaborate with interdisciplinary teams to improve continuity of care and client outcomes

  • Support families navigating dementia care, chronic illness management, caregiver stress, hospitalization recovery, and safety concerns

  • Promote individualized care solutions that allow seniors to safely remain at home while receiving high-quality support and oversight

Sales & Strategic Growth

  • Lead outreach and business development initiatives to support census growth and market expansion

  • Execute referral development and territory growth strategies

  • Track referral activity, growth metrics, and partnership opportunities

  • Collaborate with leadership to strengthen community visibility and organizational growth

  • Deliver presentations and educational programs related to holistic home care, dementia support, aging-in-place solutions, and coordinated care management services

Clinical & Operational Leadership

  • Utilize nursing expertise to support quality care delivery and clinical integrity

  • Assist with operational process improvement and service delivery optimization

  • Support compliance, quality assurance, and best practices within senior care operations

  • Participate in leadership discussions, strategic planning, and organizational development initiatives

  • Contribute to scalable systems supporting long-term organizational growth

Qualifications

  • Active Registered Nurse (RN) license required; GA licensure required

  • Minimum of 3–5 years of experience in healthcare sales, community relations, care management, business development, or healthcare leadership preferred

  • Experience in live-in care, geriatrics, dementia care, home care, hospice, rehabilitation, senior living, or care coordination strongly preferred

  • Strong entrepreneurial mindset with strategic and operational thinking abilities

  • Exceptional communication, networking, and relationship-building skills

  • Ability to work independently while contributing collaboratively to organizational growth

  • Passion for senior advocacy, aging services, and improving quality of life for older adults and families

  • Valid driver’s license and ability to travel locally for community engagement activities

Compensation & Benefits

  • Base Salary: $82,000 – $100,000 annually based on experience, qualifications, and licensure
