Licensed Practical Nurse (LPN) - Per Diem


Visiting Nurse & Hospice of Litchfield County is seeking experienced Per Diem LPNs to join our clinical teams serving Litchfield County and the Greater Farmington Valley. Must have availability for weekends and one holiday per year.


Duties include performing home visits; observe, document, and report patient status; and coordinate care with multi-disciplinary team. The Agency utilizes Netsmart for its EMR and follows National Best Practice Guidelines for caseload and visit productivity.


QUALIFICATIONS:

  • Current license as a Licensed Practical Nurse in Connecticut

  • Valid driver’s license

  • Reliable transportation and willing to travel in

  • Strong communication and interpersonal skills

  • Minimum 1- year Home health & Hospice experience
