Intermittent Care Home Health Nurse (RN) – Medicare & Medicaid

Location: Naples, Florida
Job Type: Per Diem / Intermittent Visits

Job Description

Complete Care Home Health Agency, Inc. is seeking a compassionate and dependable Registered Nurse (RN) to provide intermittent skilled nursing services to Medicare and Medicaid patients in the Naples, Florida area. The RN will provide high-quality home health care in accordance with physician orders, Medicare Conditions of Participation, Medicaid guidelines, and agency policies.

Responsibilities
  • Provide skilled nursing visits to Medicare and Medicaid patients in their homes.
  • Perform patient assessments and develop individualized plans of care.
  • Administer medications, injections, wound care, catheter care, and other skilled nursing services.
  • Complete OASIS assessments, recertifications, resumptions of care, transfers, and discharge documentation as required.
  • Educate patients and caregivers on medications, disease management, safety, and treatment plans.
  • Monitor patient progress and report significant changes to physicians and supervising clinicians.
  • Coordinate care with therapists, home health aides, physicians, case managers, and caregivers.
  • Complete accurate and timely electronic documentation in WellSky/Kinnser.
  • Maintain compliance with Medicare, Medicaid, AHCA, HIPAA, and agency standards.
  • Participate in case conferences and quality assurance activities as needed.
Qualifications
  • Active Florida Registered Nurse (RN) license.
  • Minimum one (1) year nursing experience preferred.
  • Home health experience preferred.
  • OASIS experience preferred.
  • Current BLS/CPR certification.
  • Valid Florida driver’s license and reliable transportation.
  • Ability to work independently and manage multiple patient visits efficiently.
