What You'll Do:
  • Monitor resident health and wellness through assessments, observation, and timely follow-up
  • Supervise and support Resident Care Assistants to ensure quality care and service delivery
  • Assist the Director of Resident Life (DRL) with coordinating resident care and support services
  • Complete assessments, reassessments, and individualized service plans in collaboration with residents, families, and care team members
  • Coordinate resident transitions, appointments, transportation, referrals, and related documentation
  • Oversee medication management, wellness services, and compliance with state regulations and community standards
  • Assess, document, and follow up on resident incidents, accidents, and changes in condition
  • Partner with residents, families, and interdisciplinary team members to promote resident well-being, safety, and quality of life
What You'll Need:
  • Current and active RN license in New Jersey (required)
  • 1+ years of nursing experience preferred
