We are seeking a motivated and skilled Physical Therapist (PT) to join our team. This is a per diem or contract position focusing on home health visits, with the potential to develop into part-time or full-time employment offering additional opportunities in Remote Therapeutic Monitoring (RTM) and Outpatient Physical Therapy. The ideal candidate will deliver high-quality care to patients in the comfort of their homes while adhering to Medicare and Medicaid standards.
Key Responsibilities:
  • Provide physical therapy assessments, interventions, and care planning for home health patients.
  • Perform admissions, evaluations, routine visits, and discharges per Medicare and Medicaid guidelines.
  • Educate patients and caregivers on therapy plans, techniques, and home exercise programs.
