Key Responsibilities
  • Conduct comprehensive psychosocial assessments to understand patients’ mental health status, strengths, and needs, ensuring tailored support for each individual.
  • Provide evidence-based therapeutic interventions, such as cognitive-behavioral therapy or dialectical behavioral therapy, to aid patients in coping with the emotional aspects of terminal illnesses.
  • Collaborate effectively with a multidisciplinary team of psychiatrists, nurses, chaplains, and other healthcare professionals to coordinate and optimize care for individuals and families.
  • Offer guidance and education to patients and their families regarding diagnoses, treatment options, and available resources, fostering informed decision-making.
  • Facilitate individual, group, and family therapy sessions, utilizing proven therapeutic modalities to enhance communication and emotional expression.
  • Develop personalized treatment plans in collaboration with your team, integrating patient and family goals to ensure holistic care.
  • Assist patients in navigating the complex mental health and hospice systems, advocating for their rights and ensuring their voices are heard.
  • Coordinate and facilitate psychoeducation sessions and therapeutic groups, as well as participate in discharge planning meetings, to promote continuity of care.
Patient Population / Client Focus

The primary focus of this role is on patients receiving hospice care, a sensitive and complex patient population facing life-limiting conditions. You will have the privilege of working with individuals and families experiencing a range of emotional challenges, from grief and loss to anxiety and existential concerns. Your expertise in social work will allow you to make a significant impact on their quality of life, providing them with essential support and guidance during this critical time.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) in Massachusetts.
