Key Responsibilities

As the Licensed Master Social Worker (LMSW), your core responsibilities will include:

• Conducting thorough clinical assessments to evaluate the psychosocial needs of patients and their families, and developing individualized treatment plans that reflect evidence-based practices.
• Implementing various therapeutic modalities, such as cognitive-behavioral therapy (CBT) and solution-focused psychotherapy, to support clients in coping with grief, loss, and other mental health challenges.
• Maintaining comprehensive and accurate clinical records, ensuring confidentiality in compliance with HIPAA standards, while providing timely documentation of patient progress and interventions.
• Collaborating with a multidisciplinary treatment team to develop a coordinated approach to care, participating in regular case consultations to enhance patient outcomes.
• Providing education and support to patients and their families about mental health issues, available resources, and coping strategies as they face the complexities of hospice care.
• Engaging in continuous professional development through attending workshops, webinars, and training that focus on the latest trends and developments in palliative and hospice care.

Patient Population / Client Focus

In this role, you will engage with a diverse range of patients, primarily focusing on individuals dealing with terminal illnesses and their families. Your work will be centered on providing emotional support, counseling, and advocacy, empowering clients to maintain dignity and quality of life during their care journey.

Qualifications

• A Master’s Degree in Social Work from an accredited institution is required, reflecting your commitment to professional excellence.
• A valid license to practice as a Licensed Master Social Worker (LMSW) in New York state is essential, ensuring you meet the professional standards necessary for this role.
• At least 3 years of clinical experience in social work, particularly in hospice, palliative care, or related fields, providing you with the expertise needed for this sensitive work.
• Demonstrated proficiency in therapeutic interventions tailored to individual client needs, with a strong focus on solution-based practices.
• Excellent communication and interpersonal skills, enabling you to establish rapport with clients and families troubled by grief and loss.

Preferred Experience

• Experience working within a hospice setting, understanding the unique challenges faced by patients and their families.
• Knowledge of community resources that can benefit clients and families accessing end-of-life care.
• Proficiency in using electronic medical records (EMR) systems for documentation and communication purposes.

Compensation and Benefits

Gotham Enterprises Ltd offers a competitive salary range of USD 80,000 to 100,000 per year, commensurate with experience.
