Work Environment

This role is designed to be fully remote, allowing for flexibility in your work-life balance while effectively addressing the needs of seniors and their families. You will work closely with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate care and ensure the well-being of our clients. Regular interaction with clients and families will occur through virtual meetings, emphasizing effective communication and support.

Key Responsibilities

• Conduct comprehensive psychosocial assessments to understand the unique emotional, mental, and social needs of senior clients.
• Develop personalized care plans in collaboration with clients and their families, focusing on their specific goals and preferences.
• Provide individual counseling and emotional support, helping seniors cope with life transitions such as relocation to assisted living or dealing with loss.
• Advocate for clients to ensure they have access to necessary community resources and support services.
• Monitor clients' well-being through regular follow-ups via virtual visits, assessing their overall condition and living situations.
• Educate clients and families on available community resources and support groups to enhance social connectivity and overall quality of life.
• Maintain detailed documentation of assessments, care plans, progress notes, and other relevant client interactions in compliance with organizational policies.
• Stay updated with current best practices in social work, ensuring adherence to ethical standards and state regulations.

Patient Population / Client Focus

This position is dedicated to providing services to seniors, who often face complex challenges related to aging, such as cognitive decline, loss of independence, and social isolation. You will work closely with elderly clients from diverse backgrounds, helping them navigate the intricacies of healthcare systems, emotional health, and community resources.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the State of Virginia.
• A minimum of two years of experience in a social work role, ideally in a community or geriatric setting.
• Strong knowledge of aging-related issues and the available community resources tailored to seniors.
• Exceptional assessment and counseling skills, coupled with a deep sense of empathy and commitment to client care.
• Strong proficiency in using electronic health record (EHR) systems and various software applications pertinent to case management.

Preferred Experience

• Experience working within a remote healthcare setting, demonstrating adaptability to technology-driven workflows.
• Exposure to interdisciplinary teamwork within healthcare, particularly collaborating with nurses and other social workers.
• Familiarity with virtual communication platforms for client engagement and assessments.

Compensation and Benefits
