Key Responsibilities

• Conduct comprehensive clinical assessments to evaluate the social and psychological needs of patients within our hospice setting.
• Develop individualized treatment plans that are tailored to the specific challenges faced by patients and their families during this critical stage of life.
• Provide evidence-based therapeutic interventions, focusing on grief counseling and mental health support aimed at improving patient quality of life.
• Maintain accurate and timely clinical records and documentation, ensuring compliance with regulatory standards and facilitating smooth care transitions.
• Collaborate with a diverse multidisciplinary team, including physicians, nurses, and chaplains, to coordinate comprehensive care for patients.
• Educate and support patients and families, helping them navigate the emotional and logistical complexities of hospice care.
• Stay informed on current research and developments in mental health and hospice care to continually enhance your practice.

Patient Population / Client Focus

In this role, you will engage with a diverse patient population, including individuals facing terminal illnesses and their families. You will provide emotional and psychosocial support, addressing their unique mental health needs during this sensitive time. Fostering open communication and trust will be essential as you help patients and families cope with grief, loss, and anticipatory mourning.

Qualifications

• Master's degree in Social Work from an accredited institution, ensuring a strong theoretical foundation in social work principles.
• Valid LCSW license in the state of Connecticut, demonstrating your ability to practice at a high level within our regulatory framework.
• Extensive knowledge of solution-focused therapy and psychotherapeutic interventions relevant to grief and end-of-life care.
• Exceptional communication skills, both verbal and written, to effectively interact with patients, families, and healthcare professionals.
• Strong analytical and problem-solving abilities to address complex psychosocial challenges encountered in hospice care.

Preferred Experience

• Prior experience working in hospice, palliative care, or a related field, providing insight into the challenges faced by patients and families during end-of-life transitions.
• Demonstrable skills in collaborative care models, working effectively within multidisciplinary teams to enhance patient outcomes.
• Familiarity with electronic health record (EHR) systems for documentation and patient management.
