Work Environment

Key Responsibilities

• Conduct thorough assessments of clients’ social, emotional, and environmental challenges to determine appropriate interventions.
• Develop tailored service plans that encompass client-specific goals, intervention strategies, and methods of progress monitoring.
• Provide individual and family counseling, utilizing evidence-based therapeutic techniques to support clients as they navigate complex emotional landscapes.
• Facilitate access to community resources, including housing, healthcare, and mental health services, ensuring clients receive comprehensive support.
• Engage in crisis intervention practices, conducting risk assessments and developing safety plans to address immediate challenges faced by clients.
• Collaborate with various community agencies and organizations, leveraging these partnerships to coordinate services more effectively and advocate for client needs.
• Participate in community outreach activities to increase awareness about available services and foster connections within the community.
• Attend case conferences and team meetings to discuss ongoing cases and contribute to the agency's collective knowledge base through shared experiences.

Patient Population / Client Focus

In this role, you will predominantly work with individuals and families facing critical health challenges, particularly those receiving end-of-life care. Your efforts will enable clients and their loved ones to navigate this difficult transition with dignity and support. Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is essential.
• Demonstrated understanding of trauma-informed care principles and social determinants of health.
• Proficiency in communication skills, cultural competence, and problem-solving abilities.
• Ability to adapt to different client needs while maintaining professional boundaries and ethics.

Preferred Experience

• Prior experience in a social service agency or healthcare environment, particularly in hospice or palliative care settings.
