Job Title: Per Diem Psychiatric RN – Home Health
Position Summary:

We are seeking a compassionate, experienced, and self-motivated Field Psychiatric Registered Nurse (RN) to provide in-home psychiatric nursing services on a per diem basis. This role focuses on delivering high-quality mental health care to patients in their homes, promoting stabilization, wellness, and independence.


Responsibilities:
  • Conduct in-home psychiatric nursing assessments and evaluations.

  • Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.

  • Provide skilled psychiatric nursing care and education to patients and caregivers.

  • Monitor and document changes in patient condition, behavior, and medication response.

  • Coordinate care with other clinicians, therapists, and support services.

  • Ensure timely and accurate clinical documentation in accordance with agency policies and regulatory guidelines.

  • Educate patients and families about mental health conditions, medication compliance, and coping strategies.

  • Respond to crises or urgent patient needs as appropriate within scope of practice.

  • Maintain compliance with Medicare, state, and agency standards.

  • Current, active RN license in the state of Florida.

  • 1+ years of recent psychiatric or mental health nursing experience (home health).

  • Prior home health experience.

  • Strong assessment, clinical judgment, and communication skills.

  • Reliable transportation, valid driver's license, and auto insurance.

  • Ability to work independently and manage a flexible per diem schedule.
