Job description

Job Summary:   

Provides direct care and health teaching to community dwelling adults and adolescents in accordance with agency policies and procedures 

-  Communicates with Primary Care Nurse or Clinical Manager regarding patient status and needs

-  Attends staff meetings, in-services

* Provides case management for patients when mutually agreed upon

-  Performs all other duties that Management may determine necessary

-  Reports to Clinical Manager

QUALIFICATIONS:

Minimum Education - The Per Diem Nurse must be a registered nurse with an active license to practice in the State of Connecticut.  The Per Diem Nurse may have one of the following: (1) B.S. in Nursing; (2)  Associates Degree in nursing; (3)  R.N. from approved diploma school of nursing.

 

Minimum Experience - One year experience in acute/sub-acute or home care setting. 

 

Other Qualifications-

1)The Per Diem Nurse must possess a valid driver's license and personal vehicle.

2)Active Registered Nurse license in good standing in the State of Connecticut.  3) Provide proof of current certification in Cardiopulmonary Resuscitation from American Heart Association or American Red Cross.

4) Physical, mental and sensory abilities sufficient to meet performance criteria with or without assistive devices.
