Responsibilities

  • Deliver direct nursing care to residents following care plans and facility policies.
  • Administer medications and treatments safely and accurately within LPN scope of practice.
  • Monitor, document, and promptly report changes in resident condition to the RN/supervisor and provider.
  • Collaborate with CNAs and the interdisciplinary team to support rehab goals, discharge planning, and long-term care needs.

Requirements

  • Active, unencumbered LPN licence in the state of New Mexico.
  • Minimum 1 year recent experience in SNF, LTC, or post-acute rehab preferred.
  • Current BLS/CPR certification.
  • Comfortable working 12-hour night shifts and commuting reliably to Albuquerque, NM.
