Description

Crouse Community Center is seeking a dedicated and compassionatePer Diem Physical Therapistto join our rehabilitation team. This role is ideal for a PT who values flexibility while still contributing meaningfully to resident care in a supportive, well-coordinated long-term care environment.

As a Per Diem PT, you will provide high-quality physical therapy services that help residents maintain or improve functional mobility, independence, and overall well-being. Youโ€™ll work collaboratively with nursing, medical staff, and interdisciplinary teams to ensure each resident receives individualized, person-centered care.

What Youโ€™ll Do

  • Deliver resident-centered PT servicesincluding evaluation, treatment planning, and therapeutic interventions.

  • Participate in care planningand collaborate with nursing, medical staff, and other disciplines.

  • Document assessments and progressin accordance with regulatory and facility standards.

  • Ensure safe, effective use of therapy equipmentand maintain a clean, organized therapy environment.
