Job Summary:

We are seeking a dedicated and compassionate LPN to join our team on a per diem. As a member of our healthcare family, you will provide exceptional patient care and support to our residents, collaborating with our interdisciplinary team to ensure optimal outcomes. This is a fantastic opportunity to work in a per diem capacity, offering flexibility and autonomy in your schedule.

Responsibilities:

  • Provide direct patient care to residents, including administering medications, performing treatments, and assisting with daily living activities

  • Assist with vital signs, weight, and bathings as needed

  • Develop and implement individualized care plans, under the supervision of licensed nurses

  • Collaborate with the interdisciplinary team to ensure high-quality patient care and coordinate with other healthcare professionals to achieve optimal outcomes

  • Maintain accurate and detailed records of patient care and progress

Requirements:
