Tampa, FL Biweekly Pay Day & Night Shifts Available Trach and Vent Experience Required
Pay Range:
  • RN - $37~$40
  • LPN - $34~$37

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care.
This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management.
As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.

What You'll Do
  • Provide skilled pediatric nursing 1:1 care in patients' homes
  • Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies
  • Support hospital-to-home transitions for high-acuity patients
  • Educate and support parents, families, and caregivers
  • Assist with discharge support and care coordination
  • Train and support permanent field nurses during transitions
  • Provide first-shift and interim coverage as needed
  • Collaborate with Clinical Care Managers and interdisciplinary teams
  • Respond appropriately in emergency situations and changes in patient condition
Qualifications
Required:
  • Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (FL)
  • Current CPR certification
  • Ability to lift and assist pediatric patients as needed
  • Pediatric home health experience
  • Experience with Trach care, Ventilator management, and TPN
  • High-acuity pediatric patients
Schedule & Travel
  • Full-time position, with guaranteed hours
  • Day and Night shifts available
  • On call and Staffing coverage responsibilities as needed
  • Field-based role with local travel
