Job description

Setting Overview

  • Group Home Environment
  • 1 Nurse : 6 Patients (Level 2 consumers)
  • Team-Based Model: CNA/DSP/PCA/HHA support staff
  • Respiratory support (CRT/RRT) on most shifts
  • Clinical Support Nurse overseeing 3 homes

Candidates must:

  • Be able to lift 50 lbs without limitation, restriction
  • Not be under physician work restrictions
  • Participate in Disaster Team assignments
  • Be willing to work at any assigned group home location

Scheduling Expectations

  • 12-hour shifts 6am-6pm or 6pm-6am available (Rotating weekends – NO set schedule)
  • No vacation permitted during contract
  • Must work assigned schedule and any picked-up shifts
  • Schedules completed 30+ days in advance by ADON/DON
  • Call-out policy: Minimum 4 hours prior to shift. Anything less is considered a no call/no show and grounds for termination
  • Mandatory monthly Zoom meetings (camera on – paid)

Time & Attendance Standards

  • 15 minutes early punch-in required
  • No grace period (1 minute late = tardy)

All nursing documentation (approximately 2 hours of notes) must be completed. If notes are incomplete, hours worked will not be verified to the agency.

Requirements

Licensure & Compliance Requirements

  • Current and active LPN or RN license (Compact/Multi-State accepted)
  • ACHA Level 2 Background Screening
  • Current BLS (AHA or Red Cross)
  • Current TB/PPD
  • Compliance with all local/state regulations

Experience Requirements

  • Minimum 1 year experience in acute care, long-term care (LTR), or rehab facility
  • Pediatric experience (preferred)
  • G-Tube experience (preferred)
  • Trach & Vent experience (nice to have – training available)
